Medical requirements for USF international students International students need to plan to submit their medical history, and to arrange health insurance before attending USF St. Petersburg. Health-related matters are administered by our colleagues at Student Health Services in USF Tampa. All forms can be faxed from International Affairs in St. Petersburg, or from your fax machine -- there's no need for you to travel to Tampa. Medical History and Immunization Records Student Health Services must receive your Medical History/Immunization Form before you can register for classes. You can fax the completed form to +1 813 974 5888, or mail it to: Student Health Services Health insurance forms Student Health Services will place a hold on your registration until you agree to get health insurance. To agree to get insurance you should complete the agreement form. If you do not buy adequate insurance, or if you do not file a compliance form during the first week of classes, you will be charged for USF's health insurance. USF offers one plan; other private plans are available on the open market. Some private plans meet USF's requirements and are less expensive than the USF plan. If you decide to buy another private plan, please make sure: a. that the private plan will meet the minimum requirements, b. that the start date on your insurance on the private plan is before the date you are required to have coverage, and c. that you do not file the compliance form late.
