Effective July 1, 2008 - New On-Line Version of the Internal Froam

The Proposal Authorization Form also known as the " Internal Form"is available online effective July 1, 2008. This electronic version streamlines the review process and allows the investigator, co-investigator, chair, dean and Sponsored Research the ability to track signature approvals. Also we are able to maintain the integrity of the review process and fulfill the USFSP's responsibility to the sponsor. DSR (The Division of Sponsored Research) will accept the old paper hardcopy version of the form through June 30, 2008. After this date, only the on-line, electronic version will be accepted.

Registration

First time users of this process, will need to register. Click on the link to register and create a user name and password of your choice. Once this information is completed, click "Register" to go back to the log-in page. When you log in the first time, click "Remember Me" as a time saver when you revisit the site

  • One time only, enter your contact information. Future forms will be automatically populated.
  • Electronic signatures avoid the hassles of campus mail or walking the form around campus.
  • Forms in progress, pending, signed, or archived are shown. So you will always know the status.
  • Forms can be submitted at the same time as the proposal.
  • Support documents (budgets, abstracts, etc.)can be attached
